合格証明書ごうかくしょうめいしょの発行手続はっこうてつづき【2022年(ねん)3月(がつ)以前(いぜん)の試験(しけん)】

2022年3月以前の試験に合格された方は、合格証明書の発行手続きについてはこちらのメールより申請を行ってください。

• 合格証明書の発行は、試験合格者に限ります。
• 上記のメールが自動で開かない場合は、以下の宛先に下の1.記入する事項をコピーし、記入およびファイル2つを添付をして、メールを送信してください。

メール送付のながれ

メール本文には、下記をコピーして、各項目を記入し、本人を証明する書類のファイル、顔写真のファイルを添付してください。

以下の1. 2.すべてを行ってください。

項目に記載漏れ、誤りがある場合、合格証明書の発行ができませんので、ご注意ください。

1.記入する事項

以下の①～⑪の項目を記載してください。

1. 申請者名／Applicant Name
   ex.) John Smith：
2. 申請者名のフリガナ／Applicant furigana

4. 申請者のメールアドレス／E-mail address：
5. 受験番号：Examinee's No./ID：
6. 受験者氏名 (名/ミドルネーム / 姓) / Examinee name(Given Name / Middle Name / Surname)*
   ex. ) John Smith：
   • 合格証明書に記載されます。パスポートの英文表記を入力してください。
     It will be listed on the pass certificate. Please enter the English notation of your passport.
7. 受験者の生年月日
   ex.) 1990/1/1：
8. 確認用・申請者のメールアドレス／Confirm E-mail address：
9. 受験地／Examination venue：
10. 受験した試験区分：
    • 受験票に記載の「試験区分」を入力してください。
      Please enter the "Test Category" as indicated on the examination voucher.
    • 本申請1回につき、1試験の発行申請ができます。
      You can apply for one certificate issuance by submitting one application form.
    • 2つ以上の発行申請を行う場合、別に申請入力をしてください。
      If you apply for more than one certificate issuance, please enter the application form separately.

以下の①、②の2つを添付してください。

1. 本人を証明する書類／Documents to prove the identity
   • 受験地によって、有効な書類が異なります。
     Valid certificates depend on your examination venue.
     日本：在留カード
     Japan: residence card
     インドネシア：パスポート、KTP (KARTU TANDA PENDUDUK)IDカード
     Indonesia: passport, KTP (KARTU TANDA PENDUDUK)
     タイ：パスポート、国民IDカード
     Thailand: passport, Thai national ID card
     フィリピン：パスポート、国民IDカード、UMIDカード、運転免許証
     Philippines: passport, national identification card, UMID, driver’s license
     ネパール：パスポート、国民証明書
     Nepal: passport, citizenship certificate
   • 5MB以内のJPG、GIF、PNGファイルを添付してください。
     Please upload JPG, GIF, PNG files within 5MB.
2. 顔写真／Face photo
   • 5MB以内のJPG、GIF、PNGファイルを添付してください。
     Please upload JPG, GIF, PNG files within 5MB.

顔写真データの条件/ Conditions for face photo data.

• 6か月以内に撮られた写真であること
  Must be a photo taken within 6 months
• カラー写真であること
  Be a color photograph
• データ形式はJPGまたはPNGであること
  Data format must be JPG or PNG
• 写真のサイズは200 x 230ピクセル（推奨）であること
  The size of the photo should be 200 x 230 pixels (recommended size).
• 写真は明瞭で背景は無地であること
  The photo is clear and the background is plain.
• 帽子やサングラスの着用は禁止（宗教上の理由ある場合も、顔を覆うのは禁止）
  Do not wear hats or sunglasses (even if you have religious reasons, do not cover your face).
• 正面を向いた顔と頭部の全体と肩の上部が、画像の中央に収まっていること
  The front-facing face, entire head, and upper shoulders should fit in the center of the image.
• 頭部上部と髪を含む頭部下部が、画像の縦比50～69%に収まっていること
  The upper part of the head and the lower part of the head including hair should be within the aspect ratio of 50-69% of the image.
